Office Procedures and Financial Policy
Thank you for choosing our psychiatric office. Dr. Gentile and our staff are dedicated to providing you with the best possible medical care and fostering a relationship that will grow and strengthen over the years. To facilitate this relationship, we want to ensure that you have a clear understanding of our financial and office procedure policy. Please read, understand, and sign this policy statement before any treatment.
Appointments
Our office operates on an appointment-only basis. If you need to cancel, please provide at least 24 hours' notice; otherwise, the full session fee will be incurred.
Maintaining a Respectful Environment
We strive to treat our patients and their families with courtesy and respect. Equally important is ensuring that our staff is treated respectfully by our patients. We strongly believe that our staff deserves to work in an environment free from verbal and physical abuse.
Payment for Services
Dr. Gentile is out of network for all insurance providers. Payment at the time of service is mandatory, and we accept debit/credit cards, checks, and cash. Please note that credit card payments are subject to a 3% surcharge. Upon request, we can provide a medical receipt for submission to your insurance provider for reimbursement. It is your responsibility as a patient to contact your insurance provider for details. All fees are non-negotiable and nonrefundable.

- Initial Evaluation (Adult) - $700 CPT-99205
- Initial Evaluation (Minor) – $800 CPT-99205
- 31–60-minute Session – $300 CPT-99215, 90838
- 0-30-minute Session – $200 CPT-99213, 90833
- Ketamine Treatment – $700 per Infusion 96365
- Family Therapy Session - $450 CPT-90847, 90846
